Community Health Representatives
The scope of service for the Muscogee (Creek) Nation Community Health Representative (CHR) is to provide health outreach services and health promotion/disease prevention services to Native Americans within their communities. The MCN CHR Program is staffed with five administrative staff, twenty-four Generalist positions and 4 transporter positions.
About the ServiceDiabetes Prevention
The MCN Health Diabetes Prevention and Management Program works with clinicians and community resources to provide tools and education to those who have diabetes or are at risk for the disease. Five MCNDH clinics have a Program team on-site to assist patients.
About the ServiceEmployee Health Clinic
The Employee Health Clinic, located near the Okmulgee Medical Center, serves Muscogee (Creek) Nation Tribal employees, MCN Health employees and insured dependents with no out-of-pocket expense to the patient.
About the ServiceSexual Assault Nurse Exams
The SANE Program allows survivors of sexual violence to receive specialized care by Registered Nurses who have advanced education and clinical preparation in medical forensic examinations of sexual assault victims. These RNs also have the expertise to provide effective courtroom testimony should the need arise. SANE exams are provided through a joint response of both a SANE nurse and a Victim Advocate.
About the ServiceMother & Baby Program
The Muscogee Nation Mother & Baby Program addresses nonmedical issues, utilizing the Family Spirit home-visiting model, that put mother and baby at risk, such as addiction, domestic abuse and lack of secure housing and adequate food. In addition to education, in-home or Telehealth visits by a Registered Nurse allow for monitoring and early intervention of common maternal health risks.
